About the role

Remote clinical navigator (health coach) role delivering virtual chronic care support for Medicare patients enrolled in Cadence care programs. Focuses on proactive outreach, coaching, care coordination, and addressing barriers to care, with documentation in the Cadence platform and collaboration with clinical teams.

Responsibilities

  • Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions (e.g., type 2 diabetes, hypertension, cardiovascular disease).
  • Help patients execute personalized care plans through education and coaching on behavior modification, nutrition, physical activity, and self-management using evidence-based methods (motivational interviewing, SMART goals).
  • Conduct comprehensive assessments of health status, lifestyle behaviors, nutritional habits, and readiness to change.
  • Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies; perform care delivery tasks per Cadence policies under clinical supervision.
  • Monitor progress, adherence, and health outcomes via regular check-ins and remote monitoring tools.
  • Document patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers to care and connect patients with resources or internal support pathways.
  • Collaborate in real time with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack; provide feedback to improve workflows.

Qualifications

  • An active NBHWC health coaching certification is required.
  • At least 1 year of experience as a health coach is required.
  • Experience providing patient education and support to individuals managing chronic conditions is required.
  • Remote work readiness (stable high-speed WiFi) is required.
  • Fluency in Spanish is required.
  • Fluency with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to discuss AI-supported care with providers and patients while maintaining human clinical judgment.
