About the role

Remote clinical navigator role delivering virtual chronic care support for Medicare patients in Cadence care programs. Focus on proactive patient outreach, coaching, care coordination, and documenting interactions in the Cadence platform, with collaboration to escalate concerns and coordinate next steps.

Responsibilities

  • Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions (e.g., type 2 diabetes, hypertension, cardiovascular disease).
  • Help patients execute personalized care plans and achieve their goals through education and coaching on behavior modification, nutrition, physical activity, and self-management using motivational interviewing and SMART goals.
  • Conduct comprehensive assessments of health status, lifestyle behaviors, nutritional habits, and readiness to change.
  • Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies; execute care delivery tasks under clinical supervision.
  • Monitor progress, adherence, and health outcomes via regular check-ins and remote monitoring tools.
  • Document patient interactions, outcomes, and follow-up plans in Cadence platform; identify barriers to care and connect with resources or internal support pathways.
  • Collaborate in real time with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack; provide feedback to improve workflows.

Qualifications

  • An active Medical Assistant certification from an accredited association (AAMA, AMCA, AMT, NHA, NAHP, NCCT, NRCMA/NRCAMA, or AAH)
  • At least 5 years of experience as a Medical Assistant
  • Remote work readiness (stable high-speed WiFi)
  • Spanish fluency
  • Experience with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to discuss AI-supported care with providers and patients
