About the role

Remote Care Navigator role to guide older adults and their caregivers through the intake process, matching with therapists and providing empathetic, organized patient support and care coordination.

Responsibilities

  • Guide patients and families through the intake process with warmth, clarity, and professionalism
  • Coordinate care from intake to matching with the right therapist
  • Provide emotional guidance and reassurance to clients and caregivers
  • Serve as the first voice patients hear when reaching Sailor Health
  • Manage multiple patients at different stages with attention to detail
  • Utilize tools such as Google Workspace, EHR platforms (e.g., Healthie), and CRMs (e.g., HubSpot) to support workflows

Qualifications

  • Empathetic and clear communicator
  • Highly organized and detail-oriented
  • Tech-comfortable with standard tools and CRMs
  • Mission-aligned with improving care for older adults
  • Experience in healthcare, customer service, or administrative support is welcome but not required
