Remote full-time Care Navigator role for Sailor Health, serving as the first point of contact for patients and families, guiding them through intake, care coordination, and emotional support to connect with therapists and navigate the mental health care journey for older adults.

Responsibilities

  • Guide older adults and caregivers through the intake process with warmth, clarity, and professionalism
  • Coordinate care by matching patients with the right therapist and supporting the care journey
  • Serve as the first emotional point of contact for individuals seeking care
  • Maintain organized and detailed patient information through the intake and care coordination process
  • Utilize tools such as Google Workspace, EHR platforms (e.g., Healthie), or CRMs (e.g., HubSpot) as part of daily duties
  • Convey empathy, clarity, and reassurance to patients and families throughout the process

Qualifications

  • Empathetic and clear communicator
  • Highly organized and detail-oriented
  • Tech-friendly (Google Workspace, Healthie, HubSpot, or similar)
  • Mission-aligned with improving care for older adults
  • Experience welcomed but not required in healthcare, customer service, or administrative support
