About the role

Remote Care Navigator role focused on guiding older adults and their caregivers through intake, care coordination, and emotional support to connect them with the appropriate mental health care services.

Responsibilities

  • Guide patients and their families through the intake process with warmth, clarity, and professionalism.
  • Coordinate care and support patients at different stages of the process.
  • Assist in matching patients with the right therapist and care resources.
  • Serve as the first point of contact and provide emotional guidance and reassurance throughout the journey into care.

Qualifications

  • Empathetic and clear communicator
  • Highly organized and detail-oriented
  • Tech-savvy (Google Workspace, EHR platforms like Healthie, CRMs like HubSpot)
  • Mission-aligned with a passion for improving care for older adults
  • Experience in healthcare, customer service, or administrative support is welcomed but not required
