About the role

Physician role supporting Solace’s patient advocacy program, collaborating with healthcare advocates to help patients achieve health goals and address social determinants of health. Minnesota-based physicians with DEA license and HAP to build and oversee care plans for a vulnerable population.

Responsibilities

  • Learn the Solace systems, tools, technology, partners, and expectations, while also providing your unique clinical expertise in every interaction.
  • Identify and prioritize patients’ needs and maintain a streamlined care continuum.
  • Oversee patient care plans in partnership with healthcare advocates to holistically address social determinants of health, i.e. food resources, transportation access, and support at home.

Qualifications

  • Board certification in family medicine, internal medicine or other relevant speciality.
  • Strong clinical skills paired with excellent organizational skills.
  • Superior communication skills, listening and empathy in patient interactions.
  • Comfortable working with elderly patients as well as individuals affected by Social Determinants of Health.
  • Technical savvy with ability to quickly learn new systems and software.
  • Bias toward action and execution; willingness to provide feedback.
  • Ability to adapt to rapid changes in business, strategy, organization, and priorities.
