About the role

Physician role within Solace's patient advocacy-driven telehealth platform. The physician collaborates with healthcare advocates to develop and manage patient care plans, addressing social determinants of health and achieving optimal health outcomes for a vulnerable population. Missouri-based remote position requiring DEA license and HAP.

Responsibilities

  • Learn the Solace systems, tools, technology, partners, and expectations, while also providing your unique clinical expertise in every interaction.
  • Be able to identify and prioritize patients’ needs and maintain a streamlined care continuum.
  • Oversee patient care plans in partnership with healthcare advocates to holistically address social determinants of health, i.e. food resources, transportation access, and support at home.

Qualifications

  • Board certification in family medicine, internal medicine or other relevant speciality.
  • Strong clinical skills paired with excellent organizational skills.
  • Superior communication skills, as listening and empathy are the foundation for every patient interaction at Solace.
  • Experience or comfort working with elderly patients and individuals facing Social Determinants of Health.
  • Technical savviness to quickly and fluently learn new systems and software.
  • Extreme bias toward action and execution. Strong focus on improving patient outcomes and willingness to provide feedback.
  • Ability to adapt to rapid changes in business, strategy, organization, and priorities.
  • Applicants must be based in the United States.
