About the role

Nurse Practitioner position in Lucas County, OH with Porter Cares, an independent contractor role providing in-home health risk assessments and medical management within a geographic area.

Responsibilities

  • Visit members within geographic area of responsibility and conduct an in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam.
  • Perform point-of-care testing as appropriate.
  • Identify diagnoses to be used in care management by Porter Care Guides and active medical management for treatment advancement.
  • Formulate a list of current and past medical conditions using clinical knowledge and findings from the assessment.

Qualifications

  • Valid unencumbered Ohio NP license
  • 3 years of patient care experience (experience in primary care/adult/geriatric, EMR skills preferred)
  • Strong sense of empathy, compassion, and friendliness
  • Excellent written and oral communication skills with ability to assist others with sincerity, respect, and compassion
