About the role

State-wide traveling RN position in New Jersey focusing on in-home health risk assessments, medical management support, and care coordination across the state.

Responsibilities

  • Visit members within geographic area of responsibility
  • Conduct in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform point-of-care testing as appropriate
  • Identify diagnoses for care management and active medical management in support of Porter Care Guides
  • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings

Qualifications

  • Valid unencumbered New Jersey RN license
  • 3 years of patient care experience (experience in primary care/adult/geriatric, EMR skills preferred)
  • Strong empathy, compassion, and friendliness
  • Exceptional written and oral communication skills with ability to assist patients and coworkers with sincerity and respect
