About the role

Registered Nurse role in New Jersey for Porter Cares, an independent contractor, conducting in-home health risk assessments and active medical management within a defined geographic region.

Responsibilities

  • Visit members within geographic area of responsibility.
  • Conduct in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Perform evidence-based practice screening with point-of-care testing as appropriate.
  • Identify diagnoses to be used in care management by Porter Care Guides and support active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge and findings from assessments.

Qualifications

  • Valid unencumbered New Jersey RN license
  • 3 years of patient care experience (experience in primary care/adult/geriatric, and EMR skills preferred)
  • Strong empathy, compassion, and friendliness
  • Exceptional written and oral communication skills with the ability to assist others with sincerity, respect, and compassion
