About the role

State-wide traveling Nurse Practitioner position serving New Jersey patients, visiting across the geographic area and conducting in-home health risk assessments and comprehensive physical exams.

Responsibilities

  • Visit members within geographic area of responsibility and conduct in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screening with point-of-care testing as appropriate
  • Identify diagnoses for care management by Porter Care Guides and engage in active medical management to support treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings

Qualifications

  • Valid unencumbered New Jersey NP license
  • 3 years of patient care experience required (experience in primary care/adult/geriatric, EMR skills preferred)
  • Strong sense of empathy, compassion, and friendliness
  • Exceptional communication skills, written and oral, with the ability to assist patients and coworkers with sincerity, respect, and compassion

Skills

Required:Nurse Practitioner (NP) licensure in NJ
