About the role

Travel NP position across Michigan with primary focus in the Grand Rapids area; independent contractor role with on-site visit requirements and travel accommodations.

Responsibilities

  • Visit members within geographic area of responsibility and conduct an in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam.
  • Perform evidence-based practice screening including point-of-care testing (as appropriate).
  • Identify diagnoses to be used in care management by Porter Care Guides and active medical management to support treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and findings from the assessment.

Qualifications

  • Valid unencumbered Michigan NP license
  • 3 years of patient care experience (primary care/adult/geriatric experience preferred, EMR skills preferred)
  • Strong sense of empathy, compassion, and friendliness
  • Exceptional written and oral communication skills with ability to assist patients and coworkers with sincerity, respect, and compassion
