About the role

Nurse Practitioner position with Porter Cares in Worcester, MA. Independent contractor role focused on in-home health risk assessments within a defined geographic area.

Responsibilities

  • Visit members within geographic area of responsibility and conduct in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Perform evidence-based practice screening including point-of-care testing (as appropriate).
  • Identify diagnoses to be used in care management by Porter Care Guides and active medical management in treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings.

Qualifications

  • Valid unencumbered Massachusetts NP license
  • Licensed as a Medicaid Provider
  • 3 years of patient care experience (experience in primary care/adult/geriatric, and EMR skills preferred)
  • Empathy, compassion, friendliness
  • Exceptional communication skills, written and oral, with ability to assist patients and coworkers with sincerity, respect, and compassion
