About the role

Nurse Practitioner position in Middlesex, MA with Porter Cares, Independent Contractor, on-site. In-home assessments within geographic area, leveraging an evidence-based practice approach to support care management.

Responsibilities

  • Visit members within geographic area of responsibility and conduct in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam.
  • Perform point-of-care testing as appropriate.
  • Identify diagnoses to be used in care management by Porter Care Guides and provide active medical management for treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and findings from assessments.

Qualifications

  • Valid unencumbered Massachusetts NP license
  • Licensed as a Medicaid Provider
  • 3 years of patient care experience (experience in primary care/adult/geriatric, and EMR skills preferred)
  • Strong empathy, compassion, and friendliness
  • Excellent written and oral communication skills, with ability to assist patients and coworkers with sincerity, respect, and compassion
