About the role

Nurse Practitioner position with Porter Cares, expanding into Sandusky, OH. Independent contractor, on-site in-home health risk assessments and ongoing management support.

Responsibilities

  • Visit members within geographic area of responsibility.
  • Conduct in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam.
  • Perform point-of-care testing as appropriate.
  • Identify diagnoses for care management by Porter Care Guides and active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.

Qualifications

  • Valid unencumbered Ohio NP license
  • 3 years of patient care experience (primary care/adult/geriatric experience preferred)
  • EMR skills preferred
  • Strong sense of empathy, compassion, and friendliness
  • Exceptional communication skills, written and oral, with ability to assist patients and coworkers with sincerity, respect, and compassion
