About the role

Nurse Practitioner position based in Grand Rapids, MI with travel throughout Michigan, serving as part of Porter Cares’ independent contractor, in-home health risk assessments and medical management.

Responsibilities

  • Visit members within geographic area of responsibility and conduct an in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
  • Perform evidence-based screenings with point-of-care testing as appropriate.
  • Identify diagnoses for care management and assist in active medical management for treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings.

Qualifications

  • Valid unencumbered Michigan NP license
  • 3 years of patient care experience (primary care/adult/geriatric experience preferred)
  • EMR skills preferred
  • Strong empathy, compassion, and friendliness
  • Exceptional written and verbal communication skills to assist patients and coworkers respectfully
