About the role

Porter Cares is expanding into New Jersey and is looking for Registered Nurses to conduct in-home health risk assessments within their geographic coverage (Hudson, Bergen counties) as independent contractors.

Responsibilities

  • Visit members within geographic area of responsibility and conduct an in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam.
  • Perform evidence-based practice screening with point-of-care testing as appropriate, identify diagnoses for care management, and contribute to active medical management for treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and assessment findings.

Qualifications

  • Valid unencumbered New Jersey RN license
  • 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred)
  • Strong sense of empathy, compassion, and friendliness
  • Exceptional communication skills, both written and oral, with the ability to assist others with sincerity, respect, and compassion
