About the role

Porter Cares is expanding into New Jersey and is looking for Registered Nurses to conduct in-home health risk assessments and participate in care management within specified counties (Essex and Union).

Responsibilities

  • Visit members within geographic area of responsibility and conduct an in-home health risk assessment including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and a comprehensive physical exam
  • Perform evidence-based practice screening including point-of-care testing as appropriate
  • Identify diagnoses to be used in care management by Porter Care Guides and active medical management
  • Formulate a list of current and past medical conditions using clinical knowledge and the findings of assessments

Qualifications

  • Valid unencumbered New Jersey RN license
  • 3 years of patient care experience (experience in primary care/adult/geriatric, and EMR skills preferred)
  • Strong sense of empathy, compassion, and friendliness
  • Exceptional communication skills, both written and oral, with the ability to assist others with sincerity, respect, and compassion
