About the role

Registered Nurse position for Porter Cares in New Jersey, conducting in-home health risk assessments and contributing to care management within Morris, Passaic, Sussex, and Bergen counties as an independent contractor.

Responsibilities

  • Visit members within geographic area and conduct in-home health risk assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screenings including point-of-care testing (as appropriate)
  • Identify diagnoses to be used in care management by Porter Care Guides and active medical management in support of treatment
  • Formulate list of current and past medical conditions using clinical knowledge and assessment findings.

Qualifications

  • Valid unencumbered New Jersey Registered Nurse license
  • 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred)
  • Strong empathy, compassion, and friendliness
  • Exceptional written and oral communication skills with ability to assist others with sincerity, respect, and compassion
